Services in this office are not covered by OHIP.
All fees are payable in full by the patient or parent/guardian (if patient is a minor) at the time the service is provided. Payment to our office can be made by cash, Visa, MasterCard, American Express, Interact or money order (no cheques). We do not accept assignment.
We are happy to assist you in the completion of your insurance paperwork. Where possible, estimate and claims submissions can be made electronically. Please remember that insurance is a method of reimbursing the patient for fees paid to the doctor and is not a substitute for payment. Technical support services are provided by Wardasa Management Inc., a related company.
In the event of default of payment, you will be responsible for all collection costs, bank charges, attorney fees and court costs.
Your signature on your file is acknowledgement of your financial responsibility for all charges.
